In Plan Authorization, grant access to every payer you intend to credential with. Record your attestation date.

Re-attest every 120 days WITHOUT FAIL. An expired profile stalls every active payer application. CAQH emails a reminder at day 105 — but it often goes to spam. Set a recurring calendar reminder at 90 days. At each re-attest: update your malpractice policy dates, confirm all licenses are current, update any new telehealth states. Your telehealth states must exactly match your licensed states.
