Get your EIN before your Type 2 NPI
The IRS requires your business entity to legally exist before issuing an EIN. Form your LLC/PLLC first, then get the EIN, then apply for the Type 2 NPI.
Type 1 NPI — Individual (You)

Verify your NPPES record
Confirm your legal name, address, and taxonomy codes are current BEFORE applying. Mismatches cause rejections and credentialing delays.

Create a CMS I&A account
Go to nppes.cms.hhs.gov → Create or Manage an Account. Set up 2-factor authentication. Use the same name as your state license.

Start the NPI-1 application
Apply for a New NPI → select NPI for myself (Type 1). Enter legal name, DOB, gender exactly as they appear on government ID.

Enter SSN + license info
Your SSN is used for identity verification only — it is NOT made public. Add your state NP license number and state.

Enter practice location
Physical address where you see patients. This IS public in the registry. For telehealth-only: use your office address, never your home address.

Select your taxonomy code
Tells insurers your specialty. Wrong code = claim denials. Verify at taxonomy.nucc.org before selecting.

Submit and receive your NPI
Usually issued within 20 minutes to 24 hours by email. Verify it at npiregistry.cms.hhs.gov before using it on any application.
Type 2 NPI — Organization (Your Practice)

Form your LLC/PLLC with your state
Required first — you need your Articles of Organization before the IRS will issue an EIN.

Get your EIN from the IRS
Go to irs.gov → Apply for an EIN Online (Mon–Fri, 7am–10pm ET). Free, instant. Save your CP 575 confirmation permanently.

Apply for NPI for an Organization (Type 2)
Enter your legal business name exactly as it appears on your Articles of Organization, plus your EIN.

Submit and verify
Issued within 24 hours. Verify at npiregistry.cms.hhs.gov. Use both Type 1 and Type 2 NPI on every payer credentialing application.
