Platform vs Self

Headway, Alma, Grow Therapy — and the hybrid strategy that wins
The key distinction
When you credential through a platform, you credential under their group contracts — not your own. If you leave the platform, you lose those payer relationships and must re-credential independently, which takes 60–120 days per payer. Plan accordingly.
Headway
Credentialing: ~30 daysModel: Commission 20–30%Medicare: NoMedicaid: No
Pros
+ Fastest credentialing — often under 30 days
+ No monthly fee
+ Handles all billing and claim submission
+ PMHNP-specific support and network
Cons
– You pay 20–30% of every session fee
– Payer rates are lower than direct contracts
– No Medicare or Medicaid
– You don’t own the payer relationships
Alma
Credentialing: 30–60 daysModel: $125–200/monthMedicare: LimitedMedicaid: No
Pros
+ Higher per-session revenue at scale
+ Integrated EHR, scheduling, and telehealth
+ Strong NP community and peer support
+ Break-even at approximately 8 sessions/week
Cons
– Monthly fee hurts at low volume
– No Medicaid; limited Medicare
– Acquired by Spring Health (Jan 2026)
– You don’t own the payer relationships
Grow Therapy
Credentialing: 60–120 daysModel: Commission 20–30%Medicare: Yes (select)Medicaid: Yes (select)
Pros
+ Only major platform with Medicare + Medicaid
+ No monthly fee
+ Equity-focused referral matching
+ Well-capitalized and growing
Cons
– Slowest credentialing of the three
– 20–30% commission on every session
– Not available in all states
– You don’t own the payer relationships
Recommended hybrid strategy
1. Join Headway or Alma immediately — start seeing patients and generating income while you wait for direct credentialing.
2. Simultaneously submit your Medicare, Medicaid, and top 2 commercial payer applications directly.
3. As your direct credentialing activates (60–120 days), transition patients to your own contracts.
4. Once volume is stable, evaluate whether the platform’s commission is still worth the convenience vs. billing independently.
